If it is necessary to replace a student ID card, do the following:
If the card has been lost, fill out a replacement form found in the main office.
The charge is $3.00.
Pay when form is turned in or upon receipt of new card
If the card has been damaged, you may be eligable for a free card. Fill out the same replacement form found in the office.
Possibly no charge for the replacement.
Turn in the broken card with the form.
You may
print the card replacement form
