Immunizations

IMMUNIZATIONS: ATTENTION PARENTS/GUARDIANS OF ALL NEW 6TH GRADERS AND ALL NEW STUDENTS TRANSFERRING INTO MEADS MILL FROM LOCAL PRIVATE SCHOOLS AND OUT-OF-DISTRICT SCHOOLS. Please see the enclosed Michigan Department of Community Health requirements. All students entering Meads Mill for the first time must meet the requirements stated on the enclosed form. Students who have not met the immunization requirements will be excluded from school until parents provide proof otherwise.

MEDICATION: Any student taking a prescription medication at school must have the "Permission to Administer Medication" form on file in the office with the medicine (see enclosed form). No prescribed medication will be administered starting from day one without the form filled out and signed by the doctor and parent. No prescription or non-prescription medication or devices can be kept in student lockers. All medication must be kept in the office with doctor medical form (see enclosure) or parental note attached to non-prescription medicine.

End of School Year
Remember at the end of the school year to pick up any remaining medications. Any medications remaining will be disposed of.
